Job Description
This role serves as a member of the community trained to provide education and advocacy to individuals and to the community with a goal of providing health based information geared towards reducing identified health disparities. This job encompasses the responsibilities of the Children's Healthcare Worker (CHW) and in addition: ensures information on community services is current and up to date, checks routinely with agencies for updates and educates staff; orients and onboards new CHW's and coordinated CHW's schedules and patient volumes; attends webinars (ACO/CMS) to learn best practices for successful CHW interventions and education of staff; works as CHW for the CCCM (Chronic Disease and Complex Case Management) program, which typically is a higher acuity population; assists Quality Performance Program Managers in closing quality gaps on the patients being followed; assists getting the ACO and other assigned managed populations patients in for their annual or bi-annual (chronic disease patients) physician appointments.
Qualifications
Work requires an Associate's degree in a healthcare related field, preferably with a focus on community health/social work/social sciences with at least 1 year of prior experience. Bachelor's degree preferred. Bi-lingual in Spanish or Arabic highly preferred. Knowledge of and a strong desire to help the community is essential. Valid driver’s license with no pending violations and an acceptable safe driving record, as well as compliance with Qualifications to Drive a System Vehicle as well as all policies as outlined in the SJH Fleet Management and Safety Program.

St. Joseph’s University Medical Center is an academic tertiary care medical center and state designated trauma center, located on the Paterson campus, regularly accepts referrals of difficult or unusual cases from other hospitals and physicians and performs both complex and routine procedures.
The Medical Center offers a full complement of specialty and subspecialty services including ambulatory, behavioral health, cardiovascular, emergency/trauma, internal medicine, neuroscience, oncology, orthopedic/spine, rehabilitation, surgical specialty, women’s health, imaging and pediatrics.
