Description Of Position

  • Responsible for providing on-site, individual and group, health education and counseling using the principles of motivational interviewing and goal setting to moderate and high risk members of a health plan to improve their overall health and well-being. Includes employees of the Lake Charles Memorial Health System and employer partners.

Supervision: Reports directly to Manager, Human Resources Operations

Responsibilities And Duties

  • Performs basic assessments on-site with individuals including but not limited to, lipid profile, glucose testing through the cholestech operation and blood pressure, body fat analysis.
  • Reviews labs and vitals at the time of counseling sessions or health screenings and able to identify issues during counseling sessions.
  • Supports Manager with distribution of wellness program information to employer partners.
  • Demonstrates knowledge in all areas of lifestyle management, i.e., nutrition, exercise, stress management, and disease management with understanding of links to medical management.
  • Facilitates health challenges, lifestyle-oriented programs and health presentations on-site as needed.
  • Answers health care questions and guides individuals through appointment scheduling whether it be for annual physicals, illnesses or screenings.
  • Actively engages patients/clients to maintain optimal health and wellness by assisting in personal goal setting and reducing or minimizing behaviors that directly affect chronic conditions.
  • Utilizes health behavior change models such as motivational interviewing to counsel members on health education.

Education And Work Experience

  • B.S. in health related field or secondary degree in health related field required.
  • Minimum of two years’ experience with direct patient care in an individual counseling capacity; minimum of two years’ experience in a clinical setting addressing chronic conditions.
  • Working knowledge of Microsoft programs to prepare reports.
  • License required based on education and background. Valid driver's license with ability to commute to employer locations.
  • Excellent communication and analytical skills; strong working knowledge or education in nutrition, exercise and stress management; organizational skills with strong attention to detail; ability to work independently and be a self-starter. Knowledge of administrative requirements for proper documentation of patient condition including disease process, acknowledgement of teaching and follow-up care.
  • Must be flexible with work schedule which may include events outside of the normal business day
