Position Overview
The Hospice Chaplain provides emotional and spiritual support to hospice patients, families, and caregivers. This role offers spiritual assessments, individualized plans of care, and direct support tailored to each patient’s belief system and preferences.
Key Responsibilities
- Perform spiritual assessments and develop individualized plans of care.
- Offer prayer, worship, and ritual support to meet spiritual and emotional needs.
- Provide one-on-one counseling and support with advance directives, grief, and funeral planning.
- Collaborate with interdisciplinary team members and local clergy as appropriate.
- Document all interventions and update the plan of care accordingly.
- Visit with patients and families before and after passing, offering bereavement support as needed.
- Uphold agency policies and participate in ongoing education and training.
- College degree required; Master of Divinity preferred (or equivalent experience in spiritual care with at least 1 year in a healthcare setting).
- Minimum of 1 year experience as a chaplain, preferably in hospice.
- Strong knowledge of diverse spiritual beliefs and community resources.
- Valid driver’s license and reliable transportation required.
- Deep empathy, strong communication skills, and the ability to support individuals from all walks of life.
- Patient visits vary across the Dallas-Fort Worth metroplex, including Carrollton, Dallas, Fort Worth, and surrounding areas.
