Under the direction of the Program Director, the Case Manager will be responsible for managing referrals to the program, screenings, and assessments, assisting in the identification of and connection with community resources, and supporting data input and monitoring to ensure service delivery.

The employee is required to stand and walk to make home visits. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close and color vision. Work in a non-smoking environment.

Description
Under the direction of the Program Director, the Case Manager will be responsible for managing referrals to the program, screenings, and assessments, assisting in the identification of and connection with community resources, and supporting data input and monitoring to ensure service delivery.
Qualifications
- Bachelor’s degree in Social Work or a related field required; masters degree preferred.
- Two (2) years of home visitation and health education with experience conducting evaluating assessments for children required.
There are no supervisory responsibilities for this position.
Requirements
- Ensure the timely and accurate completion of screening and assessment tools by maintaining the Form to Completion, scoring screening and assessment tools, and inputting data into program databases.
- Provide a list of assessments to Parent Educators that are due for the upcoming month (at least two weeks prior)
- Provide updates on outcomes of screening/assessment tools to Parent Educators in order to identify the need of referral to community resources for additional services for families.
- Maintain and update community resources frequently accessed by Parents As Teachers staff and families.
- Develop relationships with representatives from community resources to facilitate timely access to services.
- Work with Parent Educators to monitor referral outcomes; Collaborate with Parent Educators to identify additional resources when needed.
- Collaborate with the Program Director and Program Managers to provide training on screenings and assessment tools.
- Complete documentation of screenings/assessment tools, Forms to Completion, and data input, while assuring accuracy and timeliness.
- Provide support to ProgramDirector and Program Managers in program development and quality improvement.
- Assist and coordinate with Parent Educators in the implementation of monthly Group Connection
- Manage program referrals and documentation within the required database in a timely manner according to the Tennessee Department of Health (TDOH).
- Check Personal Visit Records (PVRs) to ensure accuracy and Parents As Teachers compliance.
- Assist Program Manager with ordering materials and program supplies.
- Maintain appropriate communications with all department areas.
- Conduct Case Manager Meetings twice a month with Parent Educators.
- Serves as a role model for Parent Educators.
- Maintain, organize, and file completed assessments in client records.
- Contribute to team efforts by performing other duties as assigned.
- Strong referral system in Memphis and Fayette County resources for program participants.
- Demonstrated administrative and organizational skills.
- Ability to work a heavy workload, under pressure of conflicting deadlines and competing needs.
- Ability to perceive and deal with sensitive issues while maintaining confidentiality and objectivity.
- Record-keeping skills to maintain accurate, up-to-date documentation that enables ready access to data for outcome measurements and research.
- Ability to work as a team player to interact and assist all employees as necessary.
- Effective oral and written communication skills.
- Ability to develop and maintain respectful relationships with fellow employees and families served.
- Ability to be flexible and help where is needed
