- Valid driver’s license and weekend availability with rotating on-call duties.
- Promotes quality home health services, serving as a liaison between clients, families, and community agencies.
- Manages patient admission, conducts initial assessments, develops care plans, and monitors ongoing needs.
- Administers medications, treatments, and supervises LPNs and aides.
- Reassesses patients post-hospitalization, updates care plans, and coordinates discharge planning.
- Maintains documentation, communicates with physicians, and ensures regulatory compliance.
- Provides patient and family education on self-care, medication, and health management.
- Participates in quality management and on-call duties to support continuous care.
- Full-time and part-time roles available with benefits including a 401(k), with pay ranging from $65,000 to $75,000 annually.
