About The Role
We are seeking an experienced and compassionate Registered Nurse (RN) - MDS Coordinator to join our team. In this position, you will play a key role in ensuring accurate assessments and high-quality care planning for our residents. Qualifications:
- Associate or Bachelor’s degree in Nursing
- Active, unencumbered New Jersey RN license
- Minimum of 2 years of MDS Coordinator experience in a long-term care setting
- Prior long-term care experience preferred
- Conduct and coordinate Minimum Data Set (MDS) assessments per state and federal regulations
- Collaborate with interdisciplinary teams to gather resident data
- Ensure assessments are accurate and complete for care planning and compliance
- Stay updated on MDS-related regulations and implement changes as needed
- Participate in care planning meetings and develop personalized care plans
- Track resident progress and update assessments based on health changes
- Coordinate with administrative and nursing staff to support accurate billing
- Provide staff training on MDS processes and documentation standards
