The Case Worker plays a crucial role in providing support to clients facing a wide range of challenges. They conduct comprehensive interviews with individuals dealing with personal and family adjustments, potential foster care situations, financial struggles, employment issues, and various other concerns related to food, clothing, housing, and physical or mental impairments. Through these interviews, the Case Worker gathers important information about the clients' medical, psychological, and social factors, enabling them to assess the nature and severity of the problems. They then offer counseling services on an individual or group basis, assisting clients in developing plans to address their needs and empowering them to tap into their inner strengths and external resources for improved social functioning. The Case Worker regularly meets with clients as required by funding sources, monitoring their progress and providing guidance to modify attitudes and behaviors for positive change. Additionally, they connect clients with community resources and organizations, establishing a network of support. Administrative tasks such as completing necessary reporting forms, maintaining accurate client case files, and ensuring effective communication with supervisors and funding sources are also part of their responsibilities. The Case Worker remains vigilant about client safety, promptly notifying relevant parties of any potential safety issues.
This is a Full Time Position at 35 Hours a Week. This Position Pays $22.00 Per Hour

Education and/or Experience Required at Entry
- Associate Degree Applicants should possess an Associate Degree in Human Services or a related field. This educational background provides a foundation of knowledge and skills relevant to the role of a Case Worker.
- Experience Candidates must have at least three years of experience working with vulnerable individuals. This experience helps them understand the unique challenges faced by the individuals they will be supporting in their role as a Case Worker.
- Lived Experience While not mandatory, candidates with lived experience are preferred and encouraged to apply. Lived experience refers to personal experience with the challenges and circumstances faced by the individuals the Case Worker will be working with, which can bring a valuable perspective to the role.
- Valid N.Y.S. Driver's License
- Registered and Insured Vehicle
- N.Y.S. Child Abuse Registry Clearance
- Mobility The Case Worker should have sufficient mobility to drive to client's homes and participate in meetings at various community facilities. These locations may not always meet federal accessibility standards for disabled individuals, so the Case Worker should be adaptable to different environments.
- Physical Demands The role may involve lifting and carrying items weighing up to 50 pounds. Additionally, the Case Worker should possess manual dexterity to work with both children and adults in challenging and active environments
