Come join our Family Support Services team as a Family Resource Coordinator who will assist individuals, families and external vendors to identify goals and develop meaningful objectives to obtain those goals. You will also identify new resources and utilize existing resources to meet the clients’ needs.

Responsibilities Include

  • Act as liaison, educator, and coach to families to identify and develop client-centered community resources to meet individual needs
  • Demonstrate a working knowledge of Medicaid applications, guardianship petitions, and Mental Health Services
  • Attend individual-focused meetings as necessary
  • Demonstrate a high level of effective communication and documentation on client’s behalf
  • Facilitate PRIDE for the Family Support Council
  • Oversee Environmental Modifications process
  • Provide information pertaining to specific grants related to Autism and Down Syndrome
  • Oversee process of distribution of funds for Campership program
  • Research potential organized respite events
  • Organize events to collaborate with families and assist with creating a natural network
  • Coordinate cost-share agreements with outside agencies in accordance to the Bureau of Developmental Services (BDS) and Moore Center Services guidelines

Requirements

The ideal candidate will have:

  • Bachelor’s degree in Human Services or related field (preferred)
  • Experience working with developmentally disabled individuals
  • Proficiency in Microsoft Office Suite and database systems
  • Valid driver’s license and access to a safe operating vehicle during scheduled working hours
  • $100,000/300,000 Automobile Liability Insurance
