This position reports to the Manager of Support Services and is based in the Columbus Service Area, with a schedule Monday-Friday, 8-4:30 pm, including on-call and holiday rotation.
It involves providing counseling and supportive services to patients and families coping with terminal illness, including psychosocial assessments, grief counseling, and assistance with legal and financial matters.
Responsibilities include developing and implementing care plans, coordinating hospice services, facilitating support groups, and participating in team meetings and quality improvement efforts.
Additional duties involve community education, training volunteers, and bereavement follow-up.
Qualifications include a Master’s in Social Work (MSW), Indiana licensure as LSW or LCSW, and relevant experience, preferably in hospice or home health. Skills required are strong interpersonal, organizational, and communication abilities, with proficiency in Microsoft Office. Physical demands include occasional lifting and exposure to various environments.
