Shared Service Agent - Public Health Nurse

The City of Brockton and the towns of Abington, Avon, Stoughton, and
Whitman Health Departments have formed an intermunicipal group called
the Plymouth Norfolk 5 (PN5). The PN5 are engaged in an innovative
initiative to strengthen core local public health services, to advance
the recommendations of the 2019 Special Commission on Local and Regional
Public Health and promote healthier communities across the region. The
PN5 Shared Public Health Services group is seeking to hire a Shared
Health Agent that will be based in Brockton, to support the Health
Departments in the city of Brockton and the towns of Abington, Avon,
Stoughton, and Whitman. SALARY: \$80,000 / Annually Grant funded
position. Please note the salary indicated is firm. REPORTS TO: Works
under the general direction of the PN5 Service coordinator and Plymouth
Norfolk 5 group ESSENTIAL FUNCTIONS: Protects the public health through
the provision of a wide variety of nursing services in homes, clinic
care, office, schools, day-care centers; provides immunization and
screening services; promotes good health practices through public
education, training, and counseling programs; investigates for
communicable disease transmission and spread. Uses MAVEN to investigate
and report cases of reportable diseases. Coordinates care and referrals
as needed. Consults with other health professionals, including social
workers, Council on Aging personnel, nurses and physicians. Provides
professional nursing services, such as community health clinics for
blood pressure screening, annual flu and vaccination clinics and other
direct services as needed. Serves as liaison with community service
groups, such as Council on Aging and the Housing Authority, and with
local and regional health care providers, to coordinate programs and
services. Hosts an annual Health and Wellness Fair for the town in
coordination with the Director. Informs and updates Board of Health on
the status of communicable diseases and other public health concerns.
Performs similar or related work as required, directed or as situation
dictates. Requirements: This position requires a bachelor's degree in
nursing and an active Massachusetts RN license. Current CPR
certification is also required or done within 6 months of hire. At least
2 years of experience in public health nursing or a related field. Valid
Massachusetts motor vehicle operator's license and a dependable
