We are seeking a compassionate and dedicated Home Care Aide to provide exceptional support and care to individuals in their homes. In this vital role, you will assist clients with daily activities, monitor their health and well-being, and promote independence and comfort.

Duties & Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Provide medication reminders in accordance with each client's care plan
- Observe clients for changes in condition and report findings accurately to the appropriate care team member
- Support clients with meal preparation, cooking, and ensuring proper nutrition tailored to individual needs
- Maintain a clean and safe environment through light housekeeping tasks such as cleaning and laundry
- Document care activities, client observations, and any incidents
- Follow personalized care plans developed in coordination with healthcare professionals
Requirements & Qualifications
- Prior experience in home health, assisted living, nursing homes, or group homes is highly preferred
- Ability to observe and accurately report changes in a client's physical or cognitive condition
- Certification in CPR and First Aid is desirable but not required
- Additional training or experience in Alzheimer's or dementia care is a plus
- Ability to lift and assist clients safely and perform physically demanding tasks as needed
- Strong communication skills to effectively interact with clients, families, and care teams
- Familiarity with HIPAA regulations to ensure client confidentiality at all times
