Dementia Health Coach – GUIDE Program
Schedule:Full-Time | 40 hours/week | Monday–Friday, 8:00 AM–4:30 PM
Fully Remote (with local availability for in-office training and visits)
Overview
The GUIDE Program is expanding, and we are seeking a dedicatedDementia Health Coachto join our multidisciplinary Dementia Care team. This role is ideal for someone deeply committed to improving the lives of individuals living with dementia and their caregivers.
As a Health Coach, you will provide compassionate peer support, help families navigate complex care needs, and connect them with meaningful community resources—including respite care. You will play a central role in care coordination, ensuring that each patient–caregiver dyad receives holistic, person-centered support.
What You’ll Do
- Provide Coaching & Emotional Support— Offer empathetic, patient-centered guidance to individuals with dementia and their caregivers, including support through grief, stress, and difficult conversations.
- Connect Families to Resources— Identify and coordinate access to respite care, community programs, educational resources, and financial or social support.
- Collaborate Across Disciplines— Work closely with clinical and non-clinical team members to ensure integrated, comprehensive care planning.
- Support Care Planning— Assist with ongoing care coordination, goal-setting, and monitoring of patient and caregiver needs.
- Track Progress & Needs— Maintain organized, accurate documentation to support continuity of care and identify emerging challenges.
Ideal Candidate Will Have
- A strongpassion for dementia careand supporting caregivers with empathy, humility, and patience
- Communication skillsthat support tough conversations, grief support, and relationship‑building over the phone
- Organizational abilitiesand self‑motivation to manage multiple dyads in a remote environment
- Team‑oriented mindsetwith the ability to rely on and collaborate with others
- Technology comfortusing virtual care platforms and remote communication tools
- Situational awarenessto navigate sensitive dynamics and maintain trust
- Experience inhealth coaching or related fields(caregiving, social work, behavioral health) or a willingness to grow through training
Required Minimum Knowledge, Skills, And Abilities (KSAs)
- Education: Bachelor’s Degree preferred.
- License Certification: Valid Driver’s License required. Health Coach Certification preferred.
- Experience: Two years of experience in a social setting or health care setting or equivalent education preferred.
- Additional Skills/Requirements Required: N/A
- Additional Skills/Requirements Preferred: Bilingual. Local community resident with knowledge of resources available in the community. Proficient computer use. Knowledge of Medical terminology.
