About The Role
As a provider with Alteas Health, you will deliver pulmonary-focused care within senior care facilities, managing a consistent patient volume while maintaining quality and efficiency. Assigned facilities will be located in Naperville, Joliet, Aurora, and Bolingbrook.
This role is ideal for a provider who is comfortable working in a structured, high-productivity environment and takes pride in staying organized, decisive, and clinically sharp throughout the day. This role offers a flexible
4-day per week schedule.
What You’ll Do
- Evaluate and manage acute, chronic, and episodic conditions
- Develop and implement patient-centered care plans
- Order, perform, and interpret diagnostic testing, including spirometry
- Prescribe medications and recommend appropriate interventions
- Educate patients and facility staff on treatment plans and disease management
- Maintain accurate, timely, and compliant documentation
- Complete and oversee the patient consent process

Compensation
Compensation is competitive and commensurate with experience, qualifications, geographic location, and demonstrated expertise. A reasonable compensation range for this position is $100,000 - $115,000 annually (based on 4 day work week - 32 hours), with final offers based on individual qualifications and organizational needs.

What We’re Looking For
Minimum Requirements
- Active and unrestricted NP license in applicable state
- Reliable, insured transportation
- Minimum of 6 months of provider experience
Preferred Experience
- 2+ years in a nursing home or post-acute setting
- Cardiology/Pulmonary experience
- 2+ years of provider experience
