JOB SCOPE

This is a clinical position requiring in-depth knowledge of mental health, DSM 5-TR diagnostic criteria, and skills in crisis intervention and stabilization. As part of the ACT you must be willing to go out into the community to provide services. This position requires clinical experience, and the desire to work as part of a team.

ESSENTIAL FUNCTIONS

  • Conducts daily visits in the community, and subsequent progress notes and other required documentation in accordance with agency, ACT, and OMH standards that demonstrate evidence-based practices
  • Completes and maintains shared assigned charts such as initial assessments including Initial Service Plans and initial Comprehensive Assessments, Wellness and Safety Plans, update Service Plan Reviews with Comprehensive assessments every 6-months using evidence-based practices and assessments required, risk assessments and other documentation as required.
  • Completes individual psychotherapy with consumers using evidence-based treatment modalities.
  • Participates in daily morning meeting, and informs ACT team in daily meetings and weekly clinical meetings around family issues and incorporate findings into service plans
  • Involves family and significant others in the consumer’s treatment with the ACT team and provide psycho-education and relapse prevention based on motivational interviewing and cognitive behavioral strategies during visits.
  • Act as advocate and liaison for consumers in relationships with family, support systems, landlords and other community supports.
  • Provide 24-hour crisis intervention on-call services on a rotating basis
  • Performs other duties, consistent with the goals and objectives of the program, as directed by Team Leader.

Qualifications

EDUCATION AND EXPERIENCE

  • Master’s Degree required – MHC-LP, LMHC, LMSW, LCSW, or equivalent in licensure in NYS preferred.
  • At least one year of experience working in human services with individuals with serious mental illness and experience in work with families.
  • Must be outcome oriented in a fast-paced environment with deadlines.
