Working Hours/Shift

FT, Monday- Friday, 8:30 am 5:30 pm

Education And Experience

What you bring to SBHG (qualifications) :

(Applicants should meet any one of the following experience and education combination)

  • 4 years of experience in a social service field OR
  • Associate degree plus 2 years of experience in the social service field OR
  • Bachelor’s degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service field

Licenses and Certifications

  • Valid California Driver’s License and current automobile insurance and must maintain a driving record that meets the company driver's eligibility policy.

How You Will Make a Difference (job Overview)

This position has primary responsibility for case management services to clients and their families referred through Stars’ Transition Age Youth (TAY) Program (TAY).

Division/Program Overview

  • Outpatient, community-based program that serves youth age 18-25 with serious and persistent mental health issues
  • Program provides intensive case management, outreach and engagement, medication support, as well as mental health counseling, peer groups, and supportive services to assist TAY transition to adult life.
  • Services offered are trauma informed and culturally responsive.
