Summary
Might be eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Duties

- Major Duties include - but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
- Orders and interprets labs - tests - and diagnostics studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

- This position potentially requires flexibility in schedule
- Driving and travel for Primary Care patient care coverage purposes will be required for this position to clinics throughout the health care system
- Driving and travel may be up to 75% of the time
