Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

  • This position is being recruited as a multi-disciplined posting to be filled as either a Nurse Practitioner or Physician
  • This Advanced Practice Nurse position is located within the Primary Care Service line located at the New Bedford CBOC for the VA Providence Healthcare System
  • This position is full-time at 40 hours per week
  • The incumbent practices in Primary Care Service as a Advanced Practice Nurse (APN) to provide a full range of services emphasizing quality - timeliness - and the advancement of the VA mission
  • The APN provides advanced clinical practice - consultation - and management
  • The APN practices to the full extent of their education - training - and certification
  • Core functions are outlined in the clinical privileges that correspond with this position
  • Duties include - but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
  • Develops and implements an individualized plan to achieve optimal population health and disease management
  • Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
  • Orders and interprets diagnostic studies
  • Prescribes pharmacologic and non-pharmacologic interventions
  • Initiates referrals/consults to specialty services
  • Provides education to patients and their families
  • Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
