Summary
This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Duties
- This position is being recruited as a multi-disciplined posting to be filled as either a Nurse Practitioner or Physician
- This Advanced Practice Nurse position is located within the Primary Care Service line located at the New Bedford CBOC for the VA Providence Healthcare System
- This position is full-time at 40 hours per week
- The incumbent practices in Primary Care Service as a Advanced Practice Nurse (APN) to provide a full range of services emphasizing quality - timeliness - and the advancement of the VA mission
- The APN provides advanced clinical practice - consultation - and management
- The APN practices to the full extent of their education - training - and certification
- Core functions are outlined in the clinical privileges that correspond with this position
- Duties include - but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
- Orders and interprets diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals/consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
