Physician - PACE

We are seeking a board-certified physician licensed in Missouri to provide primary care in a PACE setting for elderly patients.

• Provide comprehensive assessments, develop individualized care plans, and coordinate with interdisciplinary teams.
• Monitor patient health, offer education, and participate in quality improvement activities.
• Must have a medical degree, board certification in Internal Medicine or Family Medicine, and experience with elderly care preferred.
• Excellent communication, teamwork, and patient-centered care skills are essential.
