Care Coordinator (Home Health Experience)

Are you compassionate, organized, and sales-minded? We’re looking for a motivated Care Coordinator to join our team in West Indianapolis.
In this role, you’ll:
- Answer inquiries and convert them into new clients
- Create personalized care plans for seniors
- Build strong relationships with clients, families and caregivers.
- Conduct quality assurance visits
- Conduct client assessments.
- Create social media content and post it weekly.
- Support onboarding new caregivers.
What We’re Looking For
- High school diploma or equivalent
- 2–4 years of related experience (preferred similar experience candidates)
- Strong sales and communication skills
- Valid driver’s license and reliable transportation
- Comfortable working some evenings/weekends
