Location: Travel throughout Aurora area (50–75% travel required)
Schedule: Monday-Friday, Day Shift
Type: Full-Time
Pay: $53,000/year
About The Role
We are seeking a compassionate and detail-oriented
Assessment Specialist to join our team. In this role, you will travel to contracted healthcare facilities to complete comprehensive Health Risk Assessments (HRAs) for health plan members. You'll collaborate with providers, document findings in the appropriate electronic medical record (EMR), and help ensure patients receive the care and resources they need.
Key Responsibilities
- Perform comprehensive Health Risk Assessments (HRAs) for health plan members.
- Complete and communicate Interdisciplinary Care Plans.
- Explain assessment procedures to patients and obtain informed consent.
- Monitor patient comfort and safety throughout the assessment process.
- Observe and document significant patient conditions or changes, notifying the Primary Care Provider (PCP) or supervisor as appropriate.
- Accurately document assessment findings using the appropriate EMR system.
- Participate in required trainings and interdisciplinary care plan meetings.
- Maintain professionalism while representing the organization at contracted facilities.
Qualifications
Required
- Certified healthcare professional or 1–2 years of experience in care coordination, case management, or a related healthcare field.
- Certification in a healthcare field or a diploma or degree in Human Services, Health Sciences, or a related discipline.
- Ability to travel 50–75% to contracted facilities.
- Strong communication, documentation, and organizational skills.
- Ability to work independently while managing multiple priorities.
- Basic computer skills and familiarity with standard office equipment.
Preferred
- Experience completing Health Risk Assessments (HRAs).
- Familiarity with electronic medical record (EMR) systems.
- Experience working with health plan members, care coordination, or case management.
- Experience in skilled nursing, post-acute care, or other healthcare settings.
