JOB SUMMARY: The Social Worker/Case Manager will work closely with the Executive Director and team members to provide comprehensive services, including case management, interviews, and mediation, to New Jersey residents seeking affordable housing options. This role involves assisting clients through the HomeSharing process, conducting intake and assessments, and facilitating successful housing matches.
RESPONSIBILITIES
1. Service Provision
· Conduct intake screenings with prospective clients (Seekers and Providers) via phone or in-person and provide all necessary information for the HomeSharing application process.
· Determine eligibility for services by:
• Conducting criminal background checks and reference checks.
• Verifying references and collateral contacts for all applicants.
• Collecting necessary financial information.
• Interviewing applicants to assess needs and preferences for HomeSharing.
· For Seekers, conduct in-office or public venue interviews; for Providers, conduct home interviews with a walk-through of the private room and shared spaces.
· Facilitate the matching process by referring Seekers to suitable Providers.
• Complete Shared Housing Agreements between Seekers and Providers.
• Mediate discussions between Providers and Seekers, ensuring a written agreement is reached.
· Provide ongoing support and follow-ups to ensure the success of the match, including regular check-ins, phone calls, and surveys.
· Offer coaching and conflict mediation as needed to resolve issues between clients.
· Make referrals to relevant agencies for additional services, ensuring holistic support for clients.
· Advocate for clients to meet housing and other basic needs.
2. Client Activities
· Assist in the implementation of client food, gift programs, and other events as scheduled.
· Provide support for other activities as needed.
3. Outreach
· Research, identify and obtain the most appropriate resources and service providers to meet clients' needs.
· Attend relevant meetings as needed.
· Actively participate in and support the agency's fundraising events.
4. Record Keeping
· Document all client interactions and case progress in case notes.
· Obtain necessary consent for the release of confidential information from clients.
· Ensure confidentiality of all client records.
· Track time spent on client contact, case management, and outreach, and submit monthly service reports.
· Prepare reports for funders as required by grant agreements.
5. Other duties as assigned.
QUALIFICATIONS
· MSW from an accredited college or university
· Minimum three years of case management experience. Life experience may be substituted for one year’s experience.
· NJ Social Work License, or willing to test for license.
· Valid NJ Driver’s License.
· Cultural sensitivity and the ability to meet the diverse language and needs of clients.
