About the Role

As a Full-time Occupational Therapist, you will be responsible for creating and implementing individualized occupational therapy plans to meet the unique needs of our residents. You will work closely with our interdisciplinary team to identify and address the physical, cognitive, and emotional needs of our residents, focusing on functional abilities and participation in daily activities. Your expertise will help our residents regain and maintain their independence, enabling them to thrive in our community.

Responsibilities

• Conduct initial and ongoing assessments to identify resident needs and develop individualized treatment plans
• Implement and modify occupational therapy interventions to address resident goals and needs
• Collaborate with other healthcare professionals to coordinate care and services
• Educate residents, families, and caregivers on functional activities and strategies to promote independence
• Document resident progress and treatment plans in accordance with regulatory requirements
• Participate in care conferences and develop treatment plans that align with resident goals
