Position Summary

We are seeking a compassionate and patient-focused Primary Care Physician (PCP) to provide comprehensive care to a senior/geriatric patient population. The role offers a collaborative, team-based environment focused on coordinated care, quality outcomes, and an exceptional patient experience.

Key Responsibilities

• Provide comprehensive primary care to senior and geriatric patients.
• Evaluate, diagnose, and treat patients according to established standards of care.
• Manage a patient panel of approximately 500 or fewer patients.
• See approximately 8-13 patients per day.
• Participate in team huddles and collaborative care planning.
• Maintain continuity of care and a high-quality patient experience.
• Complete accurate and timely EMR documentation.
• Collaborate with multidisciplinary care teams to support patient outcomes.
• Review quality outcomes and medical records with leadership.

Qualifications

• MD or DO from an accredited institution.
• Active, unrestricted Alabama medical license.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Active DEA license.
• Medicare and Medicaid provider numbers.
